2. Name of organization

4. Phone number(s) and e-mail address (if available) at which we may reach you
5. Proof of organization's nonprofit status [copy of Internal Revenue Service ruling letter granting tax exemption under Section 501(c) (3) and 509(a) of the Internal Revenue Code]
6. Summary of the organization's mission
7. Purpose of requested funds
8. If applicable, advertising deadline date and copy/layout specifications
